Pat's Home Health Care (PHHC) is committed to providing personalized in-home care to qualified clients. PHHC understands that deciding on a care giver can be a difficult decision to make. PHHC provides peace of mind during the decision-making process by thoroughly screening all caregivers that will be providing care to your loved one. Keeping your loved one safe in the comforts of their own home is our mission. PHHC will work with families and medical providers to determine which of our services best suit the client’s needs. Medicaid’s Personal Care Services Eligibility:
To qualify for PCS, an individual must have a medical condition, disability or cognitive impairment, and demonstrates unmet needs for:

Three of the five ADLs with limited hands-on assistance
-
Two ADLs, one of which requires extensive assistance

Two ADLs, one of which requires assistance at the full dependence level
PCS program eligibility is determined by an independent assessment conducted by NC Medicaid or its designee, and is provided according to an individualized service plan.
